To be able to fully assess your entitlement under the scheme we need additional financial information - please read the descriptions of the forms below then print all that are applicable to you, complete and return with your main application form.

Confirmation of pay - PSM60 [32.3KB] You need this form if you do not have a form P60 or a week 52 payslip. |

Declaration of Self Assessment Income - PSM65(SA) [34.9KB] You need this form if you are self-employed, a company director or have income from land, property or furnished lettings. |

Confirmation of Personal Pension Contributions Paid - PSM90 [30.7KB] You need this form if you are paying into a personal pension plan (NOT company pension scheme). Please note tax relief MUST be quoted. |

[64KB] NHS funded students with a disability or specific learning difficulty may be eligible to apply for additional help to cover extra costs incurred while in training as a direct result of their disability.
